After assessing all the factors which affect the well-being of the client, we create a Care Enhancement Plan that includes short-term and long-term goals for the optimal health and well-being of the client.  This becomes the foundation for robust reporting of measured, objective outcomes which track the progress of such goals. The tools and processes we employ ensure HIPAA compliance, yet allow us to integrate a medical provider's treatment plan with our Care Enhancement Plan, aggregate the client’s medical records, and manage all aspects of their care.

Our Partner Portal, a secure, web-based application, facilitates communication among family, caregivers, guardian, and trustee regarding the care of the client.
